We heard from patients, families, unpaid carers and people with learning disabilities across Neath, Port Talbot and Swansea about their experiences of leaving hospital in the Swansea Bay University Health Board area. Through our engagement, people shared what helped them feel supported, as well as the challenges that made leaving hospital more difficult.
